Case Management

A Registered Nurse Case Manager acts as a liaison between the patient, family, physician, ancillary health care staff and community agencies in an effort to coordinate appropriate discharge plans from the Medical Center. Registered Nurse Case Manager also assists patients and families in coping with psychosocial issues that relate to illness or hospitalization.
